Transition Plan — Joining
When a participant starts with you — first week, first month, first quarter, structured.
The first 12 weeks set the tone for the whole participant relationship. This plan structures those weeks so nothing important is missed — handover from a previous provider, intake, consents, first shifts with a known worker, plan alignment, an honest 6-week and 12-week check-in.
Practice Standards specifically require a planned transition, documented, communicated and effectively managed.
