An easy-read companion to the full Service Agreement (WP-02). Same content, written in short sentences, plain words and clear sections — so a participant who finds the full version hard to read can still understand and agree to what they're signing.

Practice Standards specifically require communication to be in the language and format the participant is most likely to understand. For participants with cognitive disability, low literacy, or who speak English as a second language, the easy-read version is what makes informed consent real.

Both versions get signed.
