Different from privacy consent — this is about TWO-WAY information sharing with specific people and organisations. Family members, nominees, other providers, plan managers, allied health, GP, specialists.

Each gets its own row. The participant decides what you can share with each one — and what you can't. Includes emergency exceptions and a clear consent-duration option (ongoing / until specific date / specific purpose).

Pairs naturally with intake and the master Privacy & Information Consent. Update whenever a new person needs to be added or removed.
